Liability Waiver available via the link below.
You must complete, sign
and bring with you on day of dive.
If you have an existing medical condition, you must also
download
and print the Medical Release available below.
Please complete and
sign your portion, and have your physician
do the same. Once the
form has been completed, it too must
be brought with you.
